Job Description Reporting to the Clinical Services Manager, Inpatient Surgery, Outpatient Department and Fracture & Plastics Clinics, the Patient Navigator (PN) for the Urgent Medical Clinic (UMC) at Etobicoke General Hospital (EGH) will play a critical role in developing and coordinating a multi-disciplinary approach to care, establishing partnership within and external to EGH. As part of ongoing operations, the Patient Navigator will support: triaging and prioritization of referrals; preliminary intake and initial assessment; coordination of clinic flow; collaboration with professional practice to coordinate patient care; coordination of required diagnostics, ensuring follow-up; reviewing and communicating diagnostics results; appropriate follow-up regarding specialty services and / or consults.
